Toronto Central Home and Community Care
In 2017, Community Care Access Centres integrated with the Local Health Integration Networks (LHIN) in Ontario to become the Home and Community Care Program within the LHINs. This new structure was driven and aligns to the Patients First Act, developed in response to a need to strengthen planning, health equity and engagement with patients, families and health partners. Home and community care services, support people of all ages that require care at home, school or in their community. We look forward to continued partnership and collaboration with Home and Community Care to drive high-quality, seamless patient care across the continuum. Canadian Cancer Society - Ontario Division
The Canadian Cancer Society is a national, community-based organization of volunteers whose mission is the eradication of cancer and the enhancement of the quality of life of people living with cancer. The Canadian Cancer Society:
